My dd just turned 8 yesterday, so we will be sending in our first affidavit this summer. I already have the guidebook, but wish to clarify the medical requirements to be sent with it. First, I know she will need a medical exam by her regular doctor. We plan on keeping her records here (and in the doctorŽs office)and including just a basic checklist of what was done to meet the requirements of the law. Do I also need to include an actual list of vaccinations she has received over the years or would this be included in our records and not given to them? Second, if I understand correctly, she also needs a dental exam report included since sheŽll be considered in 3rd grade. Is this right? Third, I know she will receive a basic hearing and vision screening at her doctorŽs appointment. Will this be enough to satisfy those requirements? Lastly, are there any medical requirements I missed?
