It sounds to me like you´ve covered everything. There are various legitimate ways to show that your daughter has been vaccinated. The easiest would be to have your doctor fills out the required medical form (that can be found in our Guide to PA Homeschool Law among places) during your daughter´s medical check-up. You could simply attach that form to the affidavit.
