I donīt know how all doctorsī offices do this, but our office has a sheet on the front cover of each childīs file with the immunizations. Itīs a standardized sheet that they just fill in with dates. I have the nurse make a copy each time their record is updated and submit that as it is JUST the immunizations. Itīs also nice to have for things like camp forms & the like.
