I think you will find that if you submit a medical exemption you are going to get challenged on it shot by shot, from what I have heard from friends who oppose vaccination in other settings. And if they donīt agree then the exemption wonīt be honored and you might even have a social worker knock on your door. I would prefer to keep them in the dark and not submit the information.
When we homeschooled, the affidavit we submitted had the standard wording but it said our child had been vaccinated or was entitled to a religious exemption, but we never indicated which, and the only attachment was the booklist. There was no mention at the bottom of attaching vaccination records. I think we got this wording from HSLDA. We did this for the six years we homeschooled in Lanc Co, PA under two superintendents, and nobody ever asked about it. I personally would not give them any ammunition to go after me.
If there were a polio outbreak or our child was exposed to tetanus, I would probably give the shot, but that has not happened and it is not the schoolīs business. Each year we had a physical done by the family doctor using a form we got from a homeschool supply company which did not have vaccinations mentioned on it. We folder that part over when we xeroxed the form so it did not appear ont he blank we gave the doctor. As she had the various childhood diseases, the ages at which she had them were noted and the doctor signed the form each year. I also took photographs when she had a full case of chicken pox and a full case of English Measles to substantiate having had the natural disease and therefore should be immune.
I am glad we have done this, because she has since been diagnosed with two autoimmune disorders and she had many allergies. She probably would have been the kid that reacted to everything, so we feel we did the right thing.
