Usually all that is requested with the affidavit is info on immunizations (or a letter stating that you do not do immunizations for religious reasons). Most people include the info on the other medical checkups in the *portfolio* of that school year. IF your district might want these ´up front´ now, get the REST of your affidavit in, and then make the appointments for when you can, and get that info in then. You could also ask to have your child included in the public school free dental and medical check-ups they offer.
